Each household with an individual garden plot at a CGC member garden should fill out this form each year you have a garden. Please also take a minute to read our gardening guidelines each year.
The Community Garden Coalition is required by our primary funders, the City of Columbia and Boone County, to collect socio-economic data about our gardeners and their families on an annual basis.
If you share a plot with others in your household, please share one form. If you share a plot, but not a household, please fill out the form individually.
We will share only your contact information with your garden leader; the personal data will remain confidential.
