Each household with an individual garden plot at a CGC member garden should fill out this form each year you have a garden. Please also take a minute to read our gardening guidelines each year.
We share gardener contact information with garden leaders. To help us with fundraising and grant opportunities, we ask our gardeners for demographic statistical data. This is optional and will remain confidential.
If you share a plot with others in your household, please share one form. If you share a plot, but not a household, please fill out the form individually.
